How to Become a QPPV

The QPPV performs every individual case assessment personally. The QPPV focuses exclusively on preparing periodic safety reports. The QPPV provides system-level oversight, makes or escalates safety-critical decisions and can evidence that oversight. The QPPV is responsible only for maintaining the safety database.

A file name with no date or version. An immutable audit trail showing relevant record changes and case history. A printed summary prepared immediately before inspection. A shared folder in which users can overwrite prior versions.

The QPPV may identify risks but cannot escalate them or require corrective action. The QPPV's authority, reporting lines and escalation criteria are documented, including the ability to require corrective actions or halt distribution when justified by patient safety. The QPPV reports only to the vendor performing case processing. The QPPV's decisions are valid only if approved by the marketing department.

To compare unrelated products solely for benchmarking. To test whether descriptions, records, systems and staff explanations consistently demonstrate how the PV system operates. To allow inspectors to rely only on oral explanations. To avoid reviewing source records when summary reports are available.

The PSMF is a static historical description that does not need updating after approval. The PSMF should accurately reflect actual PV practice and be maintained through controlled review and change processes. The PSMF should contain every underlying record in full and replace all SOPs. The PSMF is relevant only during a regulatory inspection.

The company has used a committee instead of allowing the QPPV to process every case. The company lacks traceable evidence that governance decisions were based on appropriate information and followed through. The company has failed to include the full text of every clinical trial report in the minutes. The company should remove signal management from the PSMF.

It allows the QPPV to replace all functional experts. It supports informed system-level oversight and credible decisions across the PV lifecycle. It is required only to satisfy internal human resources policies. It eliminates the need for written procedures and performance metrics.

Audit every vendor and process at identical intervals regardless of risk. Use a risk-based audit cycle and document findings, CAPA closure and effectiveness evidence. Audit only after a competent authority identifies a deficiency. Avoid auditing outsourced activities because contractual controls are sufficient.

Remove the deputy arrangement because delegation is not permitted. Test the deputy handover and remote access under simulated conditions and retain the exercise report. Wait until an actual emergency occurs before testing continuity. Limit the deputy to receiving routine newsletters.

The SOP alone proves that reporting obligations were fulfilled. There is insufficient evidence that the documented process operated effectively and that submissions were completed and controlled. Submission confirmations are relevant only to clinical development. Reconciliation records should be destroyed after submission.
